What to expect on your visit
We are currently providing curbside service until further notice. Please schedule an appointment for your pet, as usual. Download and complete our intake form and history form and visit concern form prior to arrival. On the appointed day and time, bring the forms and your pet to Simplicity Animal Hospital. We will meet you at your car to retrieve both the form and your pet (please put him/her in a clean carrier or on leash), or you can choose to bring your pet onto our porch. We will bring your pet inside for care; pet owners wait outside. We will contact you with questions, if necessary.
How to complete the forms
Our forms prompt you for critical information we need to assess or complete a productive veterinary visit. Please answer all questions prior to arrival to help us prevent unnecessary delays. The forms ask the nature of the problem or concern with your pet, how long your pet has experienced symptoms, how you are feeding your pet, your pet’s routines and questions you may have, etc. Download the forms.
Preparing your pet for a visit
Clean your pet prior to arrival.
If using a carrier, be sure it’s clean. Pets can become stressed when encased in a dirty carrier. Also, your pet will appreciate a towel or blanket inside their carrier for comfort.
Be sure to bring a leash for your pet, and have the leash in place prior to us retrieving them from your care. We request you bring your dog outside of the vehicle when we are ready to transport them inside. If you prefer, you may bring your pet onto our porch.
